Pledging a gift

General information:

In almost all cases, cards are pre-printed with person’s name, campus address, administrative code number and Penn State ID number. Generally, a record of last year’s contribution to United Way will be pre-printed on the card.

Pre-printed Forms:

For employees for whom pre-printed cards have not been provided, this form is downloadable here in a PDF format (opens in new window), or a form may be obtained from your Human Resources representative or the Payroll office.

Blank Forms:

IMPORTANT: If using blank cards, employees must print name, campus address and Penn State ID number or cards cannot be processed or tallied for campaign purposes.

Continuous Deductions:

Payroll deductions will be automatically made at the amount specified previously. To confirm the amount being deducted, check the "charity" box on your payroll stub. If an employee wishes to make a change they should complete a new form. The "blue" form can be used during the annual campaign. At other times, use a "green" payroll deduction card that is available from Human Resource representatives or the Payroll Office, 307 Rider Building.

Payroll deductions will generally begin in the payroll period starting after the receipt of the pledge form, according to the deadlines regularly established for payroll changes. Cards will automatically be sent via campus mail to Payroll, 307 Rider, for campaign reporting purposes.

Filling out the form:

Payroll Deduction: Enter amount to be deducted per pay period, and yearly total (amount per pay period multiplied by number of pay periods in a year.) Please indicate if pay period is monthly or bi-weekly.

Sign and date form.

Donor Choice Designation Choices:

A – (general fund)

B – a gift to up to 4 Centre County United Way agencies (specify percentages)

C – general fund minus up to 4 exceptions

D - gift to a non-United Way agency (must have 501 © (3) status and be a human service

provider) -

E – gift to another county in Pennsylvania (see code list attached - opens in new window)
